Imaging Referrals can now be downloaded and emailed directly to us by Practitioners – simply download the form below, fill in and email directly to Info@medscan.com.au The completed referral will be delivered to Medscan Merrylands. Our bookings team will contact the patient directly via phone to arrange an appointment.
